Registration is now open for the 2023-2024 school year for all student-athletes. What is registration? The Carmel High School Athletic Department uses an online platform for families to upload student-athlete physicals as well as to complete the consent forms necessary to participate in athletics. Students will be in one of two different categories. 1) You established an account in Register My Athlete last year and need to complete the new physical form for 2023-2024 and the consent form for participation. Or… 2) You are a new student-athlete or an incoming freshman, need to create an account on Register My Athlete, and need to complete the new physical form for 2023-2024 and the consent form for participation. What needs to be completed on Register My Athlete? The process is very simple. Print or view the instructions on how to register your student-athlete. All of the forms are attachments at the bottom of this page. You can start the process of registration for 2023-2024 without having a new athletic physical form for the new school year. You can go back and add/upload the physical once completed with a physician or nurse practitioner. All forms must be completed, uploaded, and agreed to in order to participate in any open gym, workout, tryout, or practice. No exceptions.
